The preliminary report on the Philadelphia jet crash found the cockpit voice recorder likely hadn't recorded audio for several years.

Firefighters from Engine 71, about a half-mile from the crash site, were among the first emergency workers to reach the scene.

Philadelphia resident Steven Dreuitt, 37, was found dead inside a vehicle after the crash. According to Dreuitt's family, his girlfriend and 9-year-old son, Ramesses Vazquez, were also in the car.
